The model structure Art. 39(2) requires the declaration to follow: a unique identification number for the packaging; the name and address of the manufacturer and, where applicable, its authorized representative; a statement that the declaration is issued under the sole responsibility of the manufacturer; identification of the packaging allowing traceability; a statement that it conforms with the relevant Union harmonization legislation, with references to the other Union acts applied; references to the harmonized standards, common specifications, or other technical specifications used; where applicable, the notified body, what it did, and the certificate it issued; additional information; and the place and date of issue with the name, function, and signature of the signatory. Art. 39 also requires the declaration to state that the requirements of Articles 5 to 12 have been demonstrated, to contain the elements of the Annex VII module, and to be kept up to date.
